Hormonal acne vs. simple acne

There are two types of acne presentations, simple and complex. Simple acne is acne due to genetics, diet, environment, stress, bacterial overgrowth, products or medications, and other lifestyle factors. Complex acne is different from simple acne in that it occurs mainly due to hormonal disorders, particularly polycystic ovarian syndrome (PCOS).


When treating acne its important to distinguish the form of acne that the patient is presenting with. This involves asking questions that help us to assess the hormonal health and history. Clues that may suggest that the acne is hormonal in nature include but are not limited to:

  • Acne that does not improve or reoccurs after Accutane use

  • Acne that improves with birth control use

  • Acne with other symptoms including menstrual irregularities and hair loss

  • Previous history of high testosterone

After confirming the type of acne then it becomes easier to treat and heal. Tools that we can use to help treat acne include dietary and lifestyle recommendations, compounded topical creams, supplements, herbs, and necessary testing to help refine the treatment plan.
